INDICATIONS

This drug is an anti-anginal.
This drug is recommended in the treatment of severe angina pectoris.
CORVARYL is not a medicine for the treatment of angina pectoris.
DOSAGE AND ADMINISTRATION

DOSAGE
The recommended dosage is 1 tablet 3 times a day in some cases of angina pectoris, the dosage may be increased to 4 tablets per day, or 16 mg of molsidomine day.
The daily dose is to be divided and adjusted for each case.

ADMINISTRATION
Orally.
CONTRAINDICATIONS

• Significant Hypotension and shock.
• Association with VIAGRA (sildenafil):
- If traitmenet by molsidomine, sildenafil must not être.utilisé ASPD,
- In case of treatment with sildenafil, molsidomine should not be used.
• This medicine is generally not recommended during lactation.
WARNINGS and PRECAUTIONS

WARNINGS
- The combination of molsidomine on VIAGRA (sildenafil) can cause large and sudden drop in blood pressure can cause faintness, fainting, or a heart attack.
- Due to the presence of lactose, this drug should not be used in case of galactosemia, malabsorption of glucose and galactose or lactase deficiency

PRECAUTIONS
- A slight hypotension is common in molsidomine. This medication should be used with caution in patients with low blood pressure, as well as in the elderly, in patients with hypovolemia, and in patients previously treated

Molsidomine is the first representative of a new anti-anginal family, sydnonimines.

- In animals:

. Molsidomine increases venous capacitance sustainable manner thereby inducing a reduction in venous return. Systemic blood pressure tends to be lowered and attached to this phenomenon reducing venous return
causes a decrease in the left ventricular wall stress and therefore the myocardial oxygen consumption.

. Furthermore, it causes a redistribution of the blood of the epicardium to endocardium, and particularly in ischemic areas.

. It induces relaxation of vascular smooth fiber, and thus causes dilation of large epicardial trunks.

. At therapeutic doses molsidomine does not affect the inotropic and increases only little or no heart rate despite the slight decrease in peripheral resistance.

. Molsidomine does not act on the calcium conductance and has no effect on the autonomic nervous system.

. Molsidomine exerts animal antithrombotic properties coronal region and is able to limit the size of experimental infarction.

- In vitro, the action of molsidomine is at two levels:

. Smooth muscle:

. relaxation of vascular smooth muscle fiber;

. without metabolic transformation involving cysteine, which does not exhibit the phenomenon of tachyphylaxis or therapeutic escape;

. accompanied by increased levels of cyclic GMP.

In general, according to available data, the time limit before administering molsidomine a patient exposed to sildenafil is not precisely known but can be estimated at a minimum of 24 hours.
- Due to the presence of lactose, this medicine is against-indicated in cases of congenital galactosemia, malabsorption of glucose and galactose or lactase deficiency.
RECOMMENDED:
This medicine is generally not recommended during breast-feeding: breastfeeding is not recommended due to the lack of data transfer in the milk.
Adverse CORVARYL
- The usually mild headache and a slight decrease in blood pressure may occur early in treatment. Headaches usually resolve spontaneously within a few days. These effects may be increased by concomitant use of other vasodilators.
A gradual dose allows to reduce the frequency.
- Exceptionally were reported orthostatic hypotension, digestive disorders, dizziness, or itching.
Use
Therapeutic indications
- Severe angina, rebellious to the usual doses of molsidomine.
- CORVARYL tablets are not suitable for the treatment of angina pectoris.
Dosage and administration
Severe angina: 1 tablet 3 times a day (12 mg of molsidomine).
4th tablet may be necessary in some rebel unstable angina (16 mg molsidomine).
Effective dose should be achieved gradually due to the risk of headache in some subjects.
The daily dose should be adjusted and allocated for the observed effectiveness and the rate of activity of the patient.

PRECAUTIONS:

- A gradual dose is recommended in patients with hepatic insufficiency.
- Devoid of any myocardial depressant effect, molsidomine may be associated with beta-blockers. It has no effect on conduction of the fabric.
